CARF Accreditation Survey Notice
PRISM Behavioral Healthcare will soon host a survey by the Commission on Accreditation of Rehabilitation Facilities (CARF) as part of our commitment to excellence and continuous quality improvement.
The purpose of the survey is to evaluate how well the organization meets internationally recognized standards for quality and service.
Persons served, family members, staff, referral sources, and members of the community are welcome to provide feedback regarding the quality of services provided by PRISM Behavioral Healthcare. Feedback may be shared by contacting contactus@prismbh.com.

Since 2013, PRISM Behavioral Healthcare has helped individuals and families overcome substance use challenges through compassionate, accessible, and personalized care. We specialize in home-based alcohol and drug outpatient counseling, bringing professional support directly to the environments where healing and change can truly take place.

PRISM Behavioral Healthcare exists to ensure that all individuals—regardless of background, financial status, or life circumstance—can access compassionate, evidence-based behavioral healthcare that promotes healing, dignity, and long-term recovery.

- Integrity – We act with ethical behavior and transparency in all matters – financial, clinical, and administrative. This includes honest documentation, truthful billing, confidentiality, and responsible stewardship of public funds (per CARF 1.A.6 & 1.H.)
- Accountability – We take ownership of our decisions and actions. Staff follow OhioMHAS regulations, CARF standards, Medicaid rules, and internal policies. Mistakes are reported, corrected, and used for continuous improvement.
- Collaboration – We work in partnership with clients, families, referral sources, courts, schools, community agencies, and one another. We value teamwork, and understand recovery requires coordinated effort.
